Your first appointment at Perio Bright Dental Implant Center is a full periodontal evaluation, and we would much rather spend that time examining your gums than watching you fill in a clipboard. Completing your paperwork before you arrive typically saves 15 to 20 minutes in the waiting room and gives our team time to verify your benefits and review your health history before Dr. Chandrasekaran sees you.
The forms we ask new patients to complete
- Patient Registration. Your contact details, date of birth, preferred pharmacy, and who we may release information to.
- Medical and Dental History. Current medications, allergies, past surgeries, and any conditions that affect healing, such as diabetes, heart valve issues, joint replacements, or blood thinners.
- Dental Insurance Information. Your plan details and subscriber information, so we can verify your periodontal and implant benefits before treatment is scheduled.
- Privacy Notice and Consent. Acknowledgement of our privacy practices and your consent to an examination and diagnostic imaging.
How to get them
Call us at 720-676-1231 and we will send your forms to you by email or text before your appointment. If you would rather fill them in on paper, arrive about 15 minutes early and our front desk team will hand you a packet and help with anything that is unclear.
What to bring with you
- A photo ID and your dental insurance card, plus your medical insurance card if you have one.
- A current list of every medication and supplement you take, including doses.
- Your referral slip, if your general dentist referred you.
- Any recent x-rays or scans, if a previous office has not already sent them to us.
A note on the medical history
Please do not leave the health history blank or skip a medication because it seems unrelated to your mouth. Blood thinners, bisphosphonates, immune-suppressing drugs, and uncontrolled diabetes all change how we plan gum surgery and implant placement. Telling us up front is what lets us keep your treatment both safe and predictable.
