Being handed a referral can feel like being told something is seriously wrong. Usually it means something more ordinary: the part of your treatment that comes next sits inside a specialist's training and equipment, and your dentist would rather it be done well than done in-house.
What a periodontist is
A periodontist is a dentist who completed three additional years of accredited residency after dental school, focused entirely on the gums, the bone that supports the teeth, and dental implants. Board certification means passing the further written and oral examinations of the American Board of Periodontology. Dr. Chandrasekaran is a board-certified periodontist, and periodontal and implant surgery is all she does — every day, rather than occasionally.
The usual reasons for a referral
- Pocket depths that are not improving. Sites that stay deep and bleed after hygiene treatment need surgical access, regeneration, or laser therapy
- Bone loss on X-rays. Vertical defects and furcation involvement need specialist treatment planning
- Gum recession. Grafting and pinhole techniques to cover roots and rebuild attached tissue
- Implants. Placement, and the grafting — socket preservation, ridge augmentation, sinus lift — that often has to come first
- A tooth that has to come out. Difficult extractions done atraumatically, preserving bone for a future implant
- Crown lengthening. Exposing enough tooth structure for your dentist to restore, or evening out a gummy smile
- A problem around an existing implant. Bleeding, bone loss, or peri-implantitis
- Medical complexity. Diabetes, blood thinners, immune conditions, or a history of head and neck radiation
What actually happens at the first visit
The first appointment is an evaluation, not surgery. We review your medical history and your dentist's notes and X-rays, chart the pocket depth at six points around every tooth, record recession, mobility, and bleeding, and take a 3D scan if implants or grafting are being considered. Then Dr. Chandrasekaran walks you through what she found, what she recommends, what the alternatives are, and what each would cost. You leave with a written plan, and nothing is done that day that you have not agreed to.
Do you still see your regular dentist?
Yes — always. We handle the periodontal or implant portion and send notes and images back to your dentist throughout. Your dentist continues your fillings, crowns, and general care, and after periodontal treatment you may alternate maintenance visits between the two offices. The relationship is a partnership, not a transfer.
