Referral Information for Patients and Referring Dentists

Most people who walk through our door were sent by their general dentist. A referral is not a sign that something has gone wrong. It means your dentist has found something that benefits from specialist training in the gums, the bone that supports your teeth, and dental implants, and Dr. Chandrasekaran is a board-certified periodontist who treats exactly that.

For patients: what a referral means

Common reasons a general dentist refers to a periodontist include gum pockets that have not responded to cleanings, gum recession, a tooth that needs to be removed and the site preserved, a site that needs bone grafting before an implant, an implant that needs placing or repairing, or a tooth that needs crown lengthening before a crown can be fitted.

What to expect at your first visit

  • A full periodontal examination, including measurement of the pocket depth around every tooth.
  • Diagnostic imaging, which may include a 3D CBCT scan when implants or grafting are being considered.
  • A clear explanation of the diagnosis, the options, and what each one costs.
  • A written treatment plan, and a report sent back to your general dentist.

You keep your general dentist. We handle the specialist portion of your care and send you back for your routine checkups and restorative work, coordinating with them throughout.

For referring offices

We welcome referrals from general dentists, hygienists, orthodontists, and physicians across Brighton and the north Denver metro. To refer a patient, call our office at 720-676-1231 or send the referral with the following, and we will contact the patient directly to schedule.

  • Patient name, date of birth, and contact number.
  • The reason for referral and the tooth or site numbers involved.
  • Recent radiographs, and a CBCT scan if one has already been taken.
  • Current periodontal charting, if available.
  • Relevant medical history, particularly anticoagulants, bisphosphonates, and diabetes control.

What you receive back

You will get a consultation report after the initial evaluation, a copy of the treatment plan, and a completion note with post-operative details and our recommended maintenance interval, so the patient returns to your chair with everything documented. If a case is urgent, call us directly and we will make room.
